THERAPEUTIC CASE MANAGER

Location: Liverpool, Sydney

Salary: $84,858 – $89,000 per annum + Super + Salary Packaging

Hours: Full Time, Permanent

*Work From Home 2 days a week*

THE ROLE

The Therapeutic Case Manager provides specialist case work and therapeutic intervention for children and young people, their families and carers, using contemporary practices, in any or all of the following areas:

  • Foster care and/or after care support
  • Voluntary out-of-home care
  • Adoptions
  • Preservation and restoration
  • Youth/OOHC

The Case Manager is responsible for providing high quality and effective services for children and young people in Out-of-Home-Care and Permanency Support Programs, supporting and developing carers, and supporting the recruitment of foster and kinship carers. You will be required to:

  • Develop case plans for young people and their families in collaboration with stakeholders and provide ongoing therapeutic intervention and case management
  • Engage with young people and families and undertake assessments to identify their support needs
  • Assist in the preservation of families to support families at risk of entering the care system
  • Adhere to mandated reporting obligations and legislative requirements
  • Maintain records for reporting and sharing of information
  • Provide mediation to families experiencing conflict
  • Network with other relevant agencies/stakeholders as required
  • Adhere to all policies, procedures, and legislative requirements relevant to the role
  • Undertake the duties of the role ensuring a person-centred approach and the culture and context of each child/young person, their family and/or community is respected

ESSENTIAL CRITERIA

  • A minimum of a Diploma in relevant discipline such as Social Work, Community Services, Psychology or Family Therapy
  • Extensive experience working with and providing quality assessment and case management services to at risk families or children/young people in out-of- home care
  • Demonstrated understanding of the principles of contemporary case management practice
  • Demonstrated conflict resolution and mediation skills and excellent interpersonal and team work skills
  • Well-developed oral and written communication skills
  • Sound computer literacy skills
  • Current Working with Children Check Clearance
  • Current and valid Driver’s Licence
  • Willingness to undergo a National Police Check
